You need to give Denplan and your present dentist
21 days written/telephone notice that you wish to end your contract.
Notice will expire on the last day of the month and your entitlement
to accident and emergency treatment under the supplementary insurance
cover will also cease at that time. |

Denplan can send you a list of Denplan dentists
in your new local area if you wish. Remember to give your new dentist
your completed Patient Leaving Form at your first visit. For your

your first appointment as this will vary from practice to practice.
Your new dentist may feel that you need
some treatment prior to accepting you as a Denplan patient, and
you may be asked to meet the costs of any such treatment.
Denplan dentists set their own fees and therefore your monthly
fee may be different with your new dentist.

If you re-register with a start date within
six months of ending your previous contract, the registration fee
will be waived. The registration form is sent to Denplan so that we
can change your details and start collecting payments on your new
dentist's behalf. You don not need to change your Direct Debit instructions
unless your bank details have changed or if you have cancelled your
previous Denplan Direct Debit. |
